Position Description
Job Summary:
Under limited supervision, performs program duties to include case coordination for individuals with disabilities or mental illness, planning and implementing activities to ensure that the needs of program individuals are met, and insuring compliance with applicable standards. Also may oversee daily activities of Direct Care Staff.

Essential Duties and Responsibilities Summary:
Other duties may be assigned as necessary. Schedule and attend Service onsite and offsite meetings as scheduled. Coordinate objectives and meet deadlines for the implementation of Service Plans/Plans of Care. Perform direct supervisory duties with staff. Communicate issues or concerns regarding individuals with appropriate team members. Monitor compliance with program and production expectations. Implement and monitor all state regulations. Enforce policies, procedures and general guidelines of LIFESPAN. Ensure community inclusion and daily living skills training. Utilize and maximize community resources. Serve as an advocate for the individuals we serve. Provide transportation to various locations.
Perform/assist with parts of various problem solving duties which occur. Serve on Health and Safety committee as requested. Perform various administrative tasks accurately and timely. Provide training to other employees. Develop, monitor and modify daily schedules. Develop and implement objectives to ensure that individual goals are being done properly. Assist with and/or oversee medication administration, facility maintenance, individual’s personal hygiene, community outings and other special events as needed.

QP qualified “Qualified Professional” means, within the mh/dd/sas system of care;
a. An individual who holds a license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of NC by the NC Board of nursing who also has four years of full time accumulated experience in mh/dd/sa with the population served; or
b. A graduate of a college or university with a Master’s degree in a human service field and has one year of full time, post graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has one year of full time, post graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
c. A graduate of a college or university with a bachelor’s degree in a human service field and has two years of full time, post bachelor’s degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has two years of full time, post bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling; or
d. A graduate of a college or university with a bachelor’s degree in a field other than human services and has four years of full time, post-bachelor’s degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has four years of full time, post bachelor’s degree accumulated supervised experience in alcoholism and drug abuse counseling.

Ability to withstand heavy lifting and the ability to transport individuals to various locations is required. Ability to document and communicate pertinent information using computer and/or paper documentation tools. Knowledge and understanding of telephone etiquette. Working knowledge of clerical practices and procedures. Ability to apply specific department rules and regulations relating to the care of the individuals. Working knowledge and ability to use technology to include computers, scanners, communication systems and other systems.
Ability to deal tactfully and courteously with a variety of individuals and groups. Working knowledge of LIFESPAN CHOICES Training Curriculum. Ability to maintain a valid driver’s license, reliable transportation and approved automobile insurance are required.
