Position Description
Job Summary:
Under limited supervision, this person is responsible for placing individuals in the most
appropriate employment setting, volunteer or community activity and for assisting him/her
on a daily basis in Mecklenburg County only.

Essential Duties and Responsibilities:
Other duties may be assigned as necessary.
Ensure individuals are referred to and have completed the Single Portal Process.
Follow VR regulations and guidelines for supported employment.
Exercise judgment in implementing goals, policies and procedures.
Participate in and provide input into individual’s PCP (Person Centered Plan).
Ensure that goals and plans are implemented.
Assist in selecting potential individuals through screening and assessment procedures.
Oversee and direct the activities of individuals while performing job duties and/or volunteer
community activities.
Assign, review and plan work for individuals, ensuring that they are placed in the most
appropriate and least restrictive job for their individual needs and interest.
Work as liaison between the community and individuals to ensure needs are met.
Provide support for individuals in a variety of ways such as assessment, job development,
training, technical assistance, monitoring safety conditions, providing transportation and
devising weekly schedules.
Establish and maintain good relationships with various individuals and agencies including
area supervisors, referring agencies, VR counselors, parents/guardians, care coordinators,
employers, etc.
Receive, review, process, file and/or submit a variety of documents and reports for the
individuals including time studies, performance ratings, time sheets, data sheets, production
sheets, professional contact sheets, accident reports, consumer satisfaction surveys, etc.
Prepare authorization billings and mental health attendance sheets.

JOB DESCRIPTION

Remain abreast of new trends by attending workshops, seminars, conferences, meetings,
etc. as needed.
Encourage the use of natural supports at the work site and/or volunteer community
activities.
Ensure confidentiality is maintained at the work site and/or any other site involved.
Operate various types of equipment including copier, calculator, computer/lap top, cell
phone, etc.
Adhere to the principles of the CHOICES Training Curriculum.
Maintain individuals’ confidentiality at all times.
Represent in a positive manner.
Be available to work flexible hours including nights, weekends and overtime as required.
Abide by, enforce and participate in the implementation and ongoing oversight of all safety
standards and regulations.

Supervisory Responsibilities:
This position has no direct employee supervisory responsibilities. However, this position is
responsible for supervising and coaching the individuals under his/her care.

Education and/or Experience
Bachelor’s degree in Human Services or related field with a minimum of two years of
experience working with individuals with developmental disabilities, or any equivalent
combination of education and experience which provides the required knowledge, skills
and abilities.
